A 42-year-old man who recently lifted a prominent object in his garage is admitted to the hospital with significant left buttock pain. Over the past day, bruising has appeared around his left buttock. Physically, he has frail hamstring strength and is highly painful above the left ischial tuberosity. The pelvis imaging reveals no signs of a bone avulsion fracture (Figure 1). Figure 2 illustrates an avulsion rupture of the conjoined tendon of the biceps femoris, semitendinosus, and semimembranosus with 5 cm of distal retraction on sagittal short-TI inversion recovery (STIR) MRI of the pelvis.
