40 year old female without any comorbidities presented to us with history of cough with scanty EXPECTORATION for 3 months, sputum AFB repeatedly negetive, so was Sputum CBNAAT, came to us for distressing cough, and occasional wheezing, her SACE level was 180 units, underwent bronchoscopy, lymphocytic BAL with transbronchial biopsy showing epithelioid granuloma, CBNAAT negative, liver biopsy was also done.a Diagnosis of pulmonary sarcoidosis was made and patient was started on prednisolone, her symptoms improved with remission of cough.
