For the past three months, a 65-year-old man has had right knee pain and fullness behind the knee. He has received at least a couple of intra-articular steroid injections and is known to have osteoarthritis in the knee. The knee has been increasingly stiff, and he has had difficulty bending it over the last two weeks. He also reports pain with full extension and the onset of lower extremity oedema in his right leg in the past week. A big popliteal cyst can be seen on both sagittal and axial magnetic resonance imaging (MRI).
