Continuous right hip pain

A 25-year-old nulliparous with complaints of continuous right hip pain, exacerbated by hip adduction and flexion, was presented to the orthopaedic clinic. Her medical and surgical history was unremarkable. She had used a combined oral contraceptive for five years and discontinued it 18 months before presentation. The orthopaedic examination was regular. The figure shows the 2×3 cm tender tense swelling lateral and superior to the pubic tubercle in the right groin. 

The patient was aware of the lump and said the size was unchanged over six months. Blood and genital tract examinations were regular. HIV, inflammatory markers and CA125 were also average. Ultrasound reports indicated a hypoechoic lesion with some internal flow on colour Doppler. The Pelvis MRI reports figure shows that a soft tissue mass is intimately related to the round ligament. The groin lesion was excised intact with an attached portion of the round ligament through a small inguinal incision. The patient was pain-free three months post-surgery.
