A 54-year-old male presented with unstable angina and recurrent syncope. Echocardiography was performed, which revealed an ejection fraction (LVEF) of 0.35 with hypokinesis of the anterior wall. Coronary angiography (CAG) was then performed, which showed a significant left main disease. The electrocardiogram (ECG) is given below.
