A 25-year-old male patient presented with complaints of weight loss and jaundice. Physical examination revealed cheilitis, pallor, and yellowish discoloration of the sclera. These findings raised suspicion of an underlying hemolytic process. Complete blood count (CBC) results revealed significant abnormalities. The white blood cell (WBC) count was decreased at 2.06x10^3/µl, below the normal range of 4.0-10.0x10^3/µl. The red blood cell (RBC) count was notably low at 1.61x10^6/µl, compared to the normal range of 4.5-5.5x10^6/µl. Hemoglobin (Hb) levels were decreased at 6.0 g/dl, falling below the normal range of 13.0-17.0 g/dl. The hematocrit (Hct) value was 18.6%, significantly lower than the normal range of 40-50%. The mean corpuscular volume (MCV) was elevated at 115.0 fl, exceeding the normal range of 83-101 fl. The mean corpuscular hemoglobin (MCH) and mean corpuscular hemoglobin concentration (MCHC) were also increased at 37.4 pg and 32.4 gm/dl, respectively, compared to their respective normal ranges. The red cell distribution width-coefficient of variation (RDW-CV) was markedly elevated at 29%, indicating increased variation in red blood cell size. The platelet count was reduced to 56x10^3/µl, below the normal range of 150-400x10^3/µl. Additionally, nucleated red blood cells (nRBCs) were observed at a rate of 0.3/100 WBC.
