Itchy and red rash

A 62-year-old man complains of having an itchy, red rash on his chest and belly. The severity of the disease has waxed and waned over about a year. He has received two separate treatments for scabies: the first was oral ivermectin, and the second was topical permethrin. Both treatments failed to provide relief. The patient used a statin and an ACE inhibitor at the time. Multiple erythematous papules, some of which are coated in an adherent scale, are seen upon physical examination.
