A 35 year old man is brought to the emergency department of a medical college because of severe shortness of breath since 8 am in the morning. At 8 am, he began to wheeze, and it has worsened to the point that he is having extreme difficulty breathing. His past medical history is significant history of asthma since childhood and he is taking medications for asthma. On examination, his temperature is 37 C, blood pressure is 185/105 mm Hg, pulse is 90/min, and respirations are 90/min. An ABG was done on 100% oxygen: PaO2 55 mm Hg, PaCO2 49 mm Hg, and pH 7.25. The decision was made to intubate the patient. After laryngoscopy, intubation, cuff inflation, and end-tidal CO2 confirmation.
