A 9-year-old boy with a history of obesity (with a BMI of 32.1 kg/m² - 99th percentile) and non-alcoholic fatty liver disease (NAFLD) presented at the paediatric gastroenterology clinic with vomiting and abdominal pain symptoms in the right upper quadrant that had been present for one week. His medical history revealed transaminitis for four years, and he had undergone a dietary regimen for two years, which was not followed. The results of an extensive workup showed that the patient had an SGPT of 327 unit/L, SGOT of 186 unit/L, and triglycerides of 208 mg/dL. Infectious, metabolic, genetic, and autoimmune causes were ruled out. Fibro-scan and ultrasound-guided core liver biopsy were performed.
