A 90-year-old physician who is actively practising often reports weight gain due to limited ability to perform physical activity involving exercise because of knee arthrosis. The patient was on treatment of 5 mg steroid for polymyalgia. Their serum creatinine levels gradually increased from 1.1 mg/dL at 70 years old to 1.45 mg/dL at 89 years and further reached 1.5 mg/dL at 90 years. Additionally, the patient reported an estimated glomerular filtration rate of 40 ml/min/1.73m². Laboratory examination revealed a 45 mg/dL urea, with no signs of proteinuria. The patient had a history of stenting and aortic valve replacement with stable IgA monoclonal gammopathy. The haemoglobin levels (12 g/dl), total proteins- (6.8 g/d), albumin -4 g/dL, and parathyroid hormone were within the normal range. The nutritional assessment revealed a protein intake of approximately 0.9–1.1 g/kg of actual body weight/day (82 kg, height 174 cm, BMI 27 kg/m²) and 1.1–1.3 g/kg of ideal body weight/day.
