A 73-year-old lady was presented to the hospital with a recurrent paroxysmal headache for more than seven years. The headaches persisted from minutes to hours and presented with moderate swelling and repetitive paroxysmal pain throughout the whole brain. The patient has H/o hypertension and types II diabetes, which is under control now. The patient described a headache occurring after palpitation.

The patient&prime;s heart auscultation was arrhythmic, heart sound intensities differed, and her pulse was deficient. Her blood pressure was 132/75 mmHg. The clinical neurological examination was negative. However, the head MRI reports were regular, as shown in the figure.

The 24-h Holter ECG showed sinus bradycardia, sinus pause, AV junctional and ventricular escape beats, paroxysmal atrial flutter, and paroxysmal atrial fibrillation. The most extended RR interval was 5.952.
