Major thrombotic complications are rare even in cases APL-AML-M3 with multiple risk factors. But in our case,a 17 year old adolescent male without any comorbidity presented with features of a pneumonia but which was later found to be a infarct and finally diagnosed to have AML-M2 by bone marrow biopsy with likely malignancy-induced activation of the clotting system which lead on to widespread thrombosis without any organomegaly. Hence even in such presentations, acute leukemia with thrombotic potential should be considered a differential.
