A 25 year old female with no cormorbidities presented with difficulty un breathing and swallowing for 4 months,and she had urinary and fecal incontinence,weakness of bilateral lower limb.patient was vitally stable .Xray suggestive of bilateral upper zone mass suspecting mediastinal mass,CECT showed  enhanced mass in the bilateral upper lobe which extending to thoracic vertebra.USG guided biopsy s/o of giant cell tumour.in view non operable stage, patient  was started on denosumab.GCT also can present as upperlobe mass
