Due to watery diarrhoea for two weeks, a 34-year-old woman with H/O microcytic anaemia was admitted to the hospital. Electrolyte replacement therapy was started immediately. Her vital parameters were normal, and ECG showed a sinus rhythm with type atrioventricular block, flat T waves associated with U waves and a long QTc interval.
An above-average transaminase (alanine and aspartate) was found in the patient (liver function test). The blood test resulted negative for the culture test. The genetic test showed positive for the human leukocyte antigen (HLA)-DQ2 test. Upper endoscopy revealed stigmata of villous atrophy at the duodenal level, and the histopathological findings showed positive for IgA anti-endomysia antibodies (EMA Test).
Hence, the diagnosis was established as celiac disease, and the patient was started on a gluten-free diet (GFD).
