White matter hyperintensities (WMH) are regions with increased signal intensity on magnetic resonance imaging (MRI) scans, commonly indicating small vessel cerebrovascular disease. Elevated WMH volume is linked to Alzheimer's disease (AD) risk and progression. AD is pathologically characterized by Aβ plaques, with neuritic plaques and neurofibrillary tangles intensifying these changes. White matter damage is seen as a comorbidity unrelated to these pathologies. However, WMH not only predicts the onset and progression of AD comparable to or even better than other biological markers but also potentially reflects vascular forms of AD pathology.1
