A Composite Serum Biomarker Index for the Diagnosis of Systemic Sclerosis–Associated Interstitial Lung Disease: A Multicenter, Observational Cohort Study

The study aimed to develop a composite serum biomarker panel for diagnosing and stratifying the risk of SSc-associated interstitial lung disease (SSc-ILD) in patients with systemic sclerosis (SSc). The researchers analyzed 28 biomarkers in 640 participants, including SSc-ILD patients, SSc patients without ILD, idiopathic pulmonary fibrosis (IPF) controls, and healthy controls. They developed a composite index using biomarkers associated with ILD and evaluated its performance in identifying ILD and SSc-ILD, as well as its correlation with lung function, radiographic disease extent, and patient quality of life. The composite index, consisting of surfactant protein D, Ca15-3, and intercellular adhesion molecule 1, was strongly associated with SSc-ILD diagnosis and improved the identification of SSc-ILD. A higher index score was also linked to worse baseline disease severity in SSc patients. This composite serum biomarker index may enhance the identification and risk stratification of ILD in SSc patients at the initial stage.
