A New Frontier in CRSwNP Treatment: Eosinophil-Targeted Therapy with Mepolizumab and Benralizumab

This article evaluates Chronic rhinosinusitis (CRS) and its subtypes, including CRS with nasal polyps (CRSwNP) and eosinophilia (eCRSwNP). Biological treatments aimed at blocking eosinophilic inflammation in CRSwNP through the use of interleukin 5 (IL5) or the interleukin 5 receptor (IL5R) have demonstrated limited efficacy. The article reviews the evidence for mepolizumab and benralizumab in CRSwNP and highlights areas for future research and therapeutic intervention. Clinical trials on these agents in CRSwNP are limited and restricted by trial design, and the blockade of IL5/IL5R alone would provide limited clinical benefit due to the complexities of the disease's pathophysiology. The article suggests that therapy aimed at several simultaneous cytokine targets may be more effective, but well-designed trials are unlikely to be forthcoming in the short term due to the financial cost and commercial conflicts of interest.
