
This meta-analysis compares clinical and angiographic outcomes of skeletonized versus pedicled internal thoracic artery in coronary artery bypass grafting (CABG). Analyzing 28 studies with 5664 skeletonized and 7434 pedicled patients, the results reveal no difference in mortality at a 4.8-year follow-up. However, the skeletonized group exhibits higher graft failure (IRR 1.87) and lower sternal wound complications (OR 0.42) compared to the pedicled group. Short-term outcomes remain similar. In conclusion, skeletonization shows a distinct profile with increased graft failure and reduced sternal complications without affecting mortality.
