This study examined the relationship between hypertensive disorders of pregnancy (HDP) and HIV infection among 300 women in Durban, South Africa. The HIV prevalence was lower (30%) among women with HDP compared to the hospital's historical seroprevalence (45%). Women living with HIV had fewer primigravid pregnancies, and HDP developed later in gestation in WLHIV compared with uninfected women. However, WLHIV had significant maternal complications, such as abruption and elevated liver enzymes. Despite this, perinatal outcomes were similar in both groups. HIV or its treatment may have a protective effect in the development of HDP, but HDP complications may be significant in WLHIV receiving treatment.
