
In the Netherlands, the routine use of neoadjuvant short-course radiotherapy for nonlocally advanced rectal cancer has decreased by 50% from 2011 to 2016. A study involving 3,152 patients revealed that the 2016 cohort, with selective radiotherapy application, showed no compromise in cancer-related outcomes. The 4-year overall survival was significantly higher at 86.4%, compared to 79.6% in 2011, indicating safe treatment de-intensification without impacting efficacy. The study underscores the importance of optimizing clinical staging and surgery for nonlocally advanced rectal cancer after the Dutch total mesorectal excision trial.
