ACL reconstruction lowers PTOA risk and delays knee replacement. A study of 1194 patients found PTOA rates were lower in ACLR patients (22%) than in nonoperative patients (64%). ACLR delayed total knee arthroplasty (TKA) and reduced PTOA risk by 11% (P < .001). Machine learning models confirmed ACLR’s protective effect, with age, BMI, and residual laxity identified as key risk factors.
