In a clinical trial of patients with refractory restless legs syndrome (RLS), the nonpharmacological therapeutic device Tonic Motor Activation (TOMAC) was investigated for its potential to enable opioid dose reduction. Participants self-administered TOMAC sessions, and opioid doses were iteratively decreased until a Clinician Global Impression of Improvement (CGI-I) score exceeded 5. Results showed that 70% of participants successfully reduced opioid dose by ≥20% with CGI-I ≤ 5. The mean percent opioid dose reduction was 29.9%, from 39.0 to 26.8 morphine milligram equivalents per day. TOMAC demonstrated the potential to reduce opioid-associated risks for refractory RLS without worsening symptoms.
