Advanced Corneal Imaging in Keratoconus

This review aims to assess the diagnostic capabilities of the latest corneal imaging devices for identifying keratoconus. Various tomographic platforms and functional biomechanical devices were studied, and a systematic search in the PubMed database identified 469 relevant citations, of which 45 met the inclusion criteria. Sensitivities for all devices and parameters ranged from 65% to 100%, with most exceeding 90%. The area under the receiver operating characteristic curve (AUC) for the detection of keratoconus ranged from 0.82 to 1.00, indicating high accuracy. However, for keratoconus suspect detection, AUCs varied from 0.66 to 0.99, with sensitivities generally below 90%. Studies using combined indices consistently reported high AUCs, suggesting the utility of integrating multiple parameters for improved accuracy in diagnosis.
