The data are lacking on the impact of the position of the abnormal fetus in twins undergoing selective termination (ST) on pregnancy outcome. Tissue maceration post‐ST of the presenting twin may lead to early rupture of membranes, amnionitis and preterm labor. The study aims to study pregnancy complications and outcomes following ST of the presenting twin compared to the non‐presenting twin. ST of the presenting twin resulted in a higher rate of adverse pregnancy outcomes than the non‐presenting twin. These data should be presented during patient consultations. In addition, if legalization permits, it should be taken into consideration in timing ST.
