In this study of 170 patients with HPV-driven oropharyngeal cancer, AI-derived tumor and lymph node volumes from CT scans showed strong, independent associations with pre-treatment ctDNA levels. These automated volumetrics outperformed traditional clinical staging (T and N stage) in predicting ctDNA burden. The findings suggest that AI-based imaging metrics could serve as practical surrogates for ctDNA in assessing tumor burden in HPV-OPSCC.
