In a groundbreaking study, amiloride, an epithelial Na+ channel blocker, demonstrated significant potential in kidney transplant recipients (KTRs) for countering sodium retention and hypertension. The study, which included KTRs with and without albuminuria, revealed that amiloride led to a substantial increase in 24-hour urinary sodium excretion and decreased body weight in both groups. However, albuminuric KTRs experienced additional benefits, including decreased nocturnal systolic blood pressure and increased 24-hour urine aldosterone excretion. These findings suggest that epithelial Na+ channel blockers hold promise in optimizing blood pressure control in KTRs.
