This report presents a case of concurrent treatment for otosclerosis and malleus fixation (OMF) using a fully endoscopic transcanal approach. Preoperative planning involved targeted transcanalar epitympanotomy with 3D CT localization. The procedure included drilling the fixed part of the malleus head, performing a 0.6 mm stapedotomy, and inserting a piston. The patient's recovery showed closure of the air-bone gap on postoperative audiogram.
