This study investigated the characteristics and outcomes of systemic therapies in Japanese patients with metastatic renal cell carcinoma (mRCC), specifically comparing patients aged ≥75 years with those aged <75 years. The analysis included 206 patients, with 47 in the ≥75 years group. Differences in clinical factors, International Metastatic RCC Database Consortium (IMDC) risk factors, and survival outcomes were assessed. The study found a higher proportion of baseline anemia in elderly patients, leading to a higher rate of IMDC intermediate/poor risk and shorter overall survival (OS) and cancer-specific survival (CSS). Elderly patients also tended to receive best supportive care (BSC) instead of second-line active treatment.
