The study constructed a simulation model to compare the outcomes of proactively treating severe non-proliferative diabetic retinopathy (NPDR) with anti-vascular endothelial growth factor (anti-VEGF) therapy versus delaying treatment until proliferative diabetic retinopathy (PDR) develops. The model showed that early treatment of severe NPDR with anti-VEGF therapy could lead to a 51.7% relative risk reduction in PDR events over 5 years and a 19.4% absolute risk reduction, with sustained blindness rates at 10 years being 4.4% for delayed treatment and 1.9% for early treatment. The study suggests that treating severe NPDR early with anti-VEGF therapy could significantly reduce PDR incidence and sustained blindness
