This study aimed to describe the pharmacokinetics of antimicrobials in critically ill adult patients receiving extracorporeal membrane oxygenation (ECMO) and determine whether current dosing regimens achieve effective and safe exposure. The study included 993 blood samples from 85 patients in six ICUs across several countries. The results showed large variations in antimicrobial concentrations, leading to poor target attainment rates for commonly used antimicrobials. The study highlights the need for individualized dosing strategies in ECMO patients.
