A systematic review assesses the effectiveness of topical antimicrobials and antiseptics in reducing ocular surface bacteria flora (OSBF) and preventing post-operative infections. While perioperative topical antimicrobials were found to reduce OSBF effectively, they were also associated with the risk of developing resistance without providing significant additional benefits compared to topical antisepsis. On the other hand, topical antiseptics before cataract surgery and IVI were strongly supported and recommended as a prophylactic measure.
