The study measured corneal epithelial thickness in 40 early-stage keratoconus and 44 normal eyes using CASIA2 AS-OCT, showing excellent reproducibility (ICC ≥ 0.95). Keratoconus eyes had thicker superior and thinner inferior epithelium with greater thickness variability. Logistic regression combining thickness variance and ratio yielded 92.1% sensitivity and 100% specificity (AUC 0.955) for keratoconus diagnosis, highlighting AS-OCT’s promise as a screening tool.
