This randomized controlled trial evaluated autologous dermal fibroblasts (ADFs) in arthroscopic rotator cuff repair (ARCR) for full-thickness tears >2 cm. Among 86 patients, the ADF group showed a significantly lower retear rate (5.6% vs. 24.3%, P = .025) compared to controls at 6 months. While both groups demonstrated improved functional scores and range of motion postoperatively, no significant differences in clinical outcomes were observed at 6 or 12 months.
