Beta-2 Transferrin Testing Shows High Sensitivity for CSF Rhinorrhea

A single-center study evaluated the diagnostic accuracy of beta-2 transferrin (B2Tf) gel electrophoresis (GE) for detecting cerebrospinal fluid (CSF) rhinorrhea in patients with unilateral thin clear rhinorrhea (UTCR). Among 149 tests conducted on 105 patients, sensitivity was 89.5%, specificity 78.3%, positive predictive value 71.8%, and negative predictive value 92.3%. Errors were linked to purulent sinusitis (23.1%), mucous contamination (11.5%), patient collection errors (11.5%), and blood contamination (3.8%).
