This study compared the biomechanical stability of two different techniques used for distal biceps tendon repair. The extracortical single-button (SB) technique, which is commonly used, was compared to the anatomic intracortical double-button (DB) technique. Cadaveric elbows were used for the study, with one elbow randomly assigned to each technique. The elbows were cyclically loaded and then fixed in a flexed position to test the repair construct. The results showed that the DB technique had less gap formation and higher construct stiffness compared to the SB technique. The ultimate load to failure was similar between the two techniques. The study suggests that the DB technique may be a viable alternative with potentially better biomechanical performance, but caution should be exercised in patients with poor bone quality.
