The study aimed to examine longitudinal 4-meter gait speed (4MGS) change in individuals with idiopathic pulmonary fibrosis (IPF) and identify a cut-point associated with adverse outcomes. The results showed that a 0.07 m/s or more decline in 4MGS over 6 months had better discrimination for adverse outcomes than changes in other measures such as 6-minute walk distance, forced vital capacity, Composite Physiologic Index or Gender Age Physiology index. Those with a substantial decline in 4MGS had an increased risk of hospitalization or death. Therefore, 4MGS change has the potential to be a surrogate endpoint for interventions aimed at modifying hospitalization or death in IPF patients.
