This case report describes a rare complication of airway obstruction secondary to chyle leak in a patient with anaplastic thyroid carcinoma (ATC) undergoing surgery after BRAF-targeted therapy. A 55-year-old male with BRAF V600E-mutant ATC showed a remarkable response to neoadjuvant dabrafenib and trametinib. Following surgery, the patient developed a chyle leak leading to laryngeal obstruction, which persisted despite initial surgical repair and medical management. Thoracoscopic ligation of the thoracic duct ultimately resolved the issue, and the patient remains disease-free on long-term therapy. This case highlights the need for vigilance in managing complications post-surgery for patients treated with BRAF inhibitors.
