Cilta-cel or Standard Care in Lenalidomide-Refractory Multiple Myeloma

In a phase 3 trial, researchers evaluated the efficacy of cilta-cel, a CAR-T cell therapy targeting B-cell maturation antigen (BCMA), in patients with relapsed or refractory multiple myeloma who were resistant to lenalidomide. The study included 419 patients who were randomly assigned to receive either cilta-cel or standard care. After a median follow-up of 15.9 months, the cilta-cel group showed significantly longer progression-free survival compared to the standard-care group (median not reached vs. 11.8 months). Additionally, more patients in the cilta-cel group achieved an overall response, complete response or better, and had no minimal residual disease. Adverse events were reported, with cytokine release syndrome being the most common. In conclusion, cilta-cel demonstrated superior outcomes compared to standard care in lenalidomide-refractory multiple myeloma patients who had received prior treatments.
