The study compared outcomes of ACL repair and reconstruction for acute ACL rupture. Ten studies (5 RCTs, 5 cohorts) involving 549 patients were analyzed. No significant differences were found in failure rate, knee laxity, or patient-reported outcomes between repair and reconstruction groups, except for a higher hardware removal rate in the repair group. ACL repair appears as a promising alternative, but larger, higher-quality studies are required to confirm its efficacy and explore additional benefits.
