UROLOGY

This study examined trimodal therapy (TMT) using a bi-fractionated split‐course hypo-fractionated radiation therapy for nonmetastatic muscle-invasive bladder cancer in elderly patients. The researchers retrospectively reviewed the characteristics and outcomes of 122 patients over 75 treated with TMT between 1990 and 2021. The primary endpoint was overall survival, and the secondary endpoints included other survival outcomes and safety. The results showed that TMT was well-tolerated and effective, with an overall survival rate of 61.7% at three years and 51.2% at five years. For patients fit for radical cystectomy, a complete histologic response was achieved for 91.7%, and the bladder conservation rate was 90.5%. The researchers concluded that TMT may be a curative option for elderly patients unfit for radical surgery and may contribute to improved survival in these patients.
