
A study investigated the role of the anterior talofibular ligament (ATFL) fascicles in ankle stability. By sectioning the ATFL in cadavers, researchers found that isolated injury to the superior fascicle resulted in increased internal rotation and anterior translation of the talus, particularly during plantarflexion. Complete ATFL sectioning led to decreased resistance in anterior translation, internal rotation, and talus inversion. Rupture of the superior fascicle alone could cause minor instability without evident clinical laxity. Patients experiencing chronic ankle symptoms without apparent instability may have isolated superior fascicle injuries, requiring careful evaluation and potential ligament repair.
