Comparative Effectiveness and Safety of Direct Oral Anticoagulants and Warfarin in Patients With Atrial Fibrillation and Chronic Liver Disease: A Nationwide Cohort Study

A retrospective cohort study was conducted on patients with atrial fibrillation and chronic liver disease who were enrolled in a US-based administrative database between 2011 and 2017. The study compared the effectiveness and safety of direct oral anticoagulants (DOACs) as a class and individually (apixaban, rivaroxaban, dabigatran) with warfarin. The results showed that DOACs as a class and apixaban had lower risks of hospitalization for ischemic stroke/systemic embolism compared to warfarin, but the risk was not significantly different between rivaroxaban and warfarin. The risk of hospitalization for major bleeding was lower for DOACs as a class, apixaban, and rivaroxaban compared to warfarin, but the risk was higher for rivaroxaban compared to apixaban. The study concluded that DOACs as a class were associated with lower risks of ischemic stroke/systemic embolism and major bleeding compared to warfarin in patients with AF and chronic liver disease, but there was variation in outcomes between individual DOACs and warfarin.
