
A comparative study aimed to assess visual outcomes between the first and second operated eyes post small-incision lenticule extraction (SMILE). 202 patients (404 eyes) underwent SMILE for myopia and myopic astigmatism correction, using the tear film mark centration method. Parameters including optical quality, decentered displacement, corneal aberrations, and modulation transfer function (MTF) values were evaluated. Results showed no significant difference in visual quality, efficacy, or safety. However, first operated eyes exhibited lower decentered displacement, reduced horizontal coma, and higher MTF values compared to the second eyes. Linear regression demonstrated a correlation between visual quality parameters and decentered displacement.
