Comprehensive management of paranasal sinus fungus balls

The clinical consensus statement provides a comprehensive management guide for paranasal sinus fungus balls (PSFB), a common form of noninvasive mycosis that is surgically treatable. The panel of 19 specialists reached consensus on the definition, diagnosis, and treatment modalities for PSFB based on current evidence. The diagnosis requires endoscopy and single imaging, whereas a contrast medium would allow for differential diagnosis. Treatment of PSFB is mandatory before sinus augmentation and recommended for symptomatic patients, immunosuppressed patients, or patients with planned immunosuppression. Watchful waiting could be considered for asymptomatic patients with chronic rhinosinusitis who are provided with appropriate advice and assessment.
