
This longitudinal study investigated copeptin dynamics and their associations with metabolic changes in 64 youth (mean age 18.7 years) with obesity undergoing metabolic and bariatric surgery (MBS, n=34) or nonsurgical (NS, n=30) lifestyle management. Over 12 months, copeptin levels increased more significantly in the MBS group (p=0.017), while HbA1c and adiposity decreased more in MBS than NS (p≤0.036). In the MBS group, copeptin changes correlated positively with HbA1c and HOMA-IR (p≤0.05) and negatively with resting energy expenditure (REE, p=0.036). These findings suggest vasopressin, reflected by copeptin, may play a role in MBS-related metabolic improvements.
