The CONTACT trial, which explored household contact management (HCM) for tuberculosis, found that community-based HCM models of care were more effective and cost-efficient than facility-based models. The study, conducted in Cameroon and Uganda, showed that community health workers increased tuberculosis preventive treatment (TPT) initiation and completion rates, resulting in a greater number of TPT courses and a reduction in tuberculosis-related deaths. The cost-effectiveness analysis indicated that the incremental cost-effectiveness ratio was $620 per disability-adjusted life-year (DALY) averted in Cameroon and $970 per DALY averted in Uganda. The findings suggest that community-based HCM is a cost-effective strategy to reduce child tuberculosis deaths.
