This study evaluates the importance of platelets in haemostasis, atherothrombosis, and regulating inflammatory and microcirculatory pathways. Antiplatelet therapies are effective in preventing thrombo-ischaemic complications in atherosclerotic diseases but also increase the risk of bleeding. The study reviewed the pathophysiological role of platelets in acute myocardial ischaemia, vascular inflammation, and atherosclerotic progression. It also highlights current clinical concepts of antiplatelet therapy and discusses novel therapeutic targets and compounds for antiplatelet therapy that may offer new strategies to treat cardiovascular disease with a more favourable safety profile than currently approved drugs.
