Definitions matter: detection rates and perinatal outcomes for infants prenatally identified with late fetal growth restriction using biometric vs consensus Delphi criteria

This study aimed to compare the screening performance of SMFM biometric (estimated fetal weight (EFW) or abdominal circumference (AC) <10th centile for gestation) versus the Delphi consensus criteria for the birth of a small for gestational age (SGA) infant at term, emergency caesarean section (CS) for non‐reassuring fetal status (NRFS), perinatal mortality, and severe neonatal morbidity. Biometric and Delphi consensus criteria collectively identified 1030 cases of late FGR. Both biometric and Delphi standards had strong screening potential for detecting infants with BW <10th or <3rd centile for gestation but not for adverse neonatal outcomes. EFW and AC had the highest likelihood ratios for predicting perinatal mortality.
