In this systematic review and meta-analysis, the diagnostic accuracy of various ultrasound signs for the detection of adnexal torsion was assessed using surgical findings as the reference standard. The study included 18 studies published between 1990 and 2021, evaluating ovarian edema, adnexal mass, ovarian Doppler flow findings, the whirlpool sign, and pelvic fluid as ultrasound signs. The quality of the studies was assessed using the QUADAS-2 tool. The results showed that while the presence of ovarian edema, the whirlpool sign, and decreased or absent ovarian Doppler flow had good specificity for detecting adnexal torsion, the presence of an adnexal mass or pelvic fluid had poor diagnostic accuracy. In addition, the heterogeneity of the analysis was high for all evaluations.
