
In a prospective pilot study of comatose survivors of cardiac arrest (CA), diffusion kurtosis imaging (DKI)-MRI was evaluated against somatosensory evoked potentials (SSEPs) to predict arousal recovery. Results revealed that DKI-MRI, especially with higher diffusion b-values, provided valuable insights into the prognosis of arousal recovery, particularly in cases where SSEPs were unavailable or unreliable. A decrease in mean diffusivity and an increase in mean kurtosis in specific brain regions were observed in subjects without arousal recovery potential, highlighting the potential of DKI-MRI in improving early predictions in post-CA care.
